Essential Items to Bring for Your DOT Physical

Getting ready for your DOT physical doesn't have to be complicated. A little preparation before your appointment can help your exam go more smoothly and may prevent delays in completing your medical certification.

Before you head to your appointment, here are a few important things to bring with you.

Bring Your Photo ID

Start with a current photo ID, such as your driver's license. Having your identification ready helps us verify your information and get your visit started.

If you have your current DOT medical card or other paperwork related to your certification, it's also a good idea to bring it with you.

Bring a List of Your Medications

If you take prescription or over-the-counter medications regularly, bring an up-to-date medication list.

Include the name of each medication, the dosage, and how often you take it. You can also bring your medication bottles if that's easier.

Some medications or medical conditions may require additional information from the healthcare provider who manages your care, so having accurate information available can help avoid unnecessary delays.

Wear Your Glasses or Contacts

If you wear prescription glasses or contact lenses for driving, bring them to your DOT physical.

Vision is part of the DOT medical examination, and you should be prepared to complete your vision screening using the corrective lenses you normally wear.

If you use both glasses and contacts, bringing your glasses as a backup isn't a bad idea either.

Don't Forget Your Hearing Aids

If you use hearing aids, bring them with you and make sure they are working properly.
